From day one we can support you

We can support you over the phone, in the comfort of your home, in the community or at one of our specialist centres.

As life changes, we’ll help you adjust.

  • We work with you to understand your needs and support you to achieve your goals so you can live well independently.
  • Our free emotional, physical, medical and practical support aims to help you be independent, stay at home and live well for as long as possible.
  • We will spend time with you and our free support also extends to your family and friends.
  • Our specialist team of care professionals come together to put you at the centre of everything we do.

We’re here for your family, too

We can help you talk confidently and openly about your health to those you love.  

Even if you don’t think you need our support right now, it’s good to get in touch early.  

There may come a point when you want advice about writing a will, counselling services, or bereavement support.  

Just being aware of your options will give you peace of mind, so you can continue enjoying your life. 

Live life to the full

Being diagnosed with a life-limiting condition doesn't change who you are.  We will help you to continue doing what you love and remain as independent as possible.

From the first day you’re diagnosed, we can be there for you and your family as much or as little as you need us.

If you’ve been diagnosed with a life-limiting condition including cancer, motor neurone disease, dementia, lung, heart, kidney or liver disease, or if you’re unsure and need to speak to someone, please call us.

All our services are completely FREE

We can help you:

  • Understand your diagnosis, and manage your symptoms, so you can get on with your life.
  • Discuss how your diagnosis may impact your benefits, work, and finances, so you get the financial support you’re entitled to.
  • Speak confidently to those you love about what you’re going through and connect your family with the services they need.   
  • Access support from physiotherapists and clinical specialists easily without a referral from your GP.  
  • Talk things over in your own time. We’re always here to chat. No problem is too big, and no worry is too small.
